What "anonymous" means here

Most survey tools call a survey anonymous when the form has no name field. The response still carries an IP address, a browser fingerprint and a timestamp that can be matched against VPN, badge or email logs. Anonymous mode removes that data before anything is saved.

Set a minimum group size of five

Do not report any cut of the data (team, tenure, location) that contains fewer than five responses. Below that, answers are identifiable by elimination.

Send one generic link

Post the link in a channel rather than emailing individual links, and do not wrap it in a tracking redirect from your email tool.

Review open-text comments before managers see them

A comment about "the Tuesday client escalation" identifies its author without a name. Generalise the detail, keep the substance.

Report back within two weeks

Publish what you heard and the two or three things you will change. Participation in the next round depends on it.

Frequently asked questions

Is it really anonymous, or just "we don't ask for your name"?

Really anonymous. When anonymous mode is on, the server discards the respondent's email, name, IP address, browser user agent and referrer before the response is saved. Only the answers and a submission date are stored. This is enforced server-side, so a modified form or API client cannot bypass it.

Can I see who responded?

No. There is no identity attached to a response in anonymous mode, and no way to reconstruct one afterwards. You see counts, averages, distributions and the text of open answers.

Can employees answer more than once?

Because nothing identifying is stored, duplicates cannot be detected. In practice this rarely matters. If it does, set a response limit close to your headcount and a closing date.

Does it work for small teams?

Yes, but be careful with questions. Any demographic question that narrows to fewer than five people, such as department in a 12-person company, can identify someone by elimination. The template avoids these by default.

Is there a free plan?
