Tips for using this template

  • Leave anonymous mode on. Turning it off later does not un-anonymise old responses, but it does break trust for the next round.
  • Do not add "Which department are you in?" unless every department has at least five people.
  • Share one generic link in a team channel rather than emailing individual links, so nobody can match link to person.
  • Publish the aggregate results and the two or three actions you will take within two weeks of closing the survey.
  • Run it on the same week each quarter so the trend line is comparable.

Frequently asked questions

Is this survey actually anonymous?

Yes. With anonymous mode on, the server discards email, name, IP address, user agent and referrer before the response is saved. It cannot be bypassed by editing the form. See the anonymous mode documentation for the full list of what is and is not stored.

Can employees answer more than once?

Because nothing identifying is stored, duplicate answers cannot be detected. In practice this is rarely a problem. If it matters, set a response limit close to your headcount and a closing date.

How many responses do I need?

For a team of 8 to 30 you want at least 70 percent participation for the averages to mean much. Above 100 people, 50 percent is usually enough to spot real patterns.
